What is known
A 12% discount was requested on a confirmed order. Dana may approve up to 10%.
Why it is in this state
The request exceeds the requester's own authority, so it waits for someone who holds it.
Who or what resolves it
Maya, sales manager. Her illustrated limit is 20%.
The safe next action
Wait for the decision. Nothing has been applied to the order, and the warehouse can see why its task is on hold.

A notification that a decision is needed is not the decision record itself.

The response was lost

What is known
The connection dropped while the confirmation was being sent. The browser never received an answer.
Why it is in this state
A lost answer is not a failed action. The work may have completed, and asking again blindly risks doing it twice.
Who or what resolves it
The system checks for the original result under the same request identity before anything else is offered.
The safe next action
Either the recorded result appears, or the state is reported honestly as unresolved with a named way to continue.

This illustrates the recovery principle the product is designed around. It is hypothetical: no real request is made here, and no system can promise that every external effect happens exactly once.

What is known
Eleven shelving units came back. Ten were accepted into stock; one was refused as damaged, with a photo and its dock location recorded.
Why it is in this state
Receiving may refuse damaged goods with evidence. The refused unit stays the customer's property and waits for their driver.
Who or what resolves it
The warehouse records the disposition; a second person checks the credit note before it is issued.
The safe next action
A credit note for the ten accepted units follows the original invoice. Nothing is refunded in cash.

Physical and financial outcomes can differ, and both stay explainable.
